Services Accepting Referrals

We accept patient referrals for the following services:

Lip Tie and Tongue Tie Assessment and Laser Frenectomy Infants with feeding difficulties, children with speech or oral function concerns, and adults with restricted lingual or labial frenulum. Assessment focuses on functional restriction. Treatment is performed using a CO₂ dental laser. We work collaboratively with the referring provider and, where appropriate, with lactation consultants, speech-language pathologists, and myofunctional therapists as part of the care plan.

Sleep Apnea and Snoring Treatment Patients with a confirmed diagnosis of obstructive sleep apnea (via sleep study) who may benefit from custom oral appliance therapy as an alternative or adjunct to CPAP. Patients with primary snoring who may be candidates for laser-assisted soft tissue treatment. We coordinate care with the referring sleep physician throughout treatment.

Therapeutic Botox for TMJ Disorders and Bruxism Patients with temporomandibular joint disorders, chronic bruxism, or muscle-tension-related facial pain who may benefit from targeted botulinum toxin therapy as part of a broader management approach.

Dental Implant Consultation Patients requiring single-tooth, multi-tooth, or full-arch implant assessment and treatment planning, including cases that may require bone grafting evaluation.

Nitrous Oxide Sedation for Patients with Medical Complexity Patients who require dental treatment with sedation support due to anxiety, medical conditions, or other considerations.

General and Preventive Dentistry We welcome referrals for patients of all ages seeking a dental home in the Sarnia area, including families, new residents, and patients transitioning from other providers.

What to Include in Your Referral

To help us prepare for your patient's visit, please include the following information in the referral form below:

  • Referring provider's name, practice, and contact information
  • Patient's name, date of birth, and contact information
  • Reason for referral and relevant clinical history
  • Any diagnostic imaging, sleep study results, or clinical notes that support the referral
  • Urgency or timeline considerations
  • Preferred method of communication for follow-up

Supporting documents such as sleep study reports, imaging, or clinical notes can be sent separately to our office by fax or secure email.

How the Referral Process Works

1. Submit the referral using the form below or by contacting our office directly.

2. We contact the patient to schedule an appropriate appointment. For infant tongue tie and lip tie referrals, we prioritize timely scheduling given the impact on feeding.

3. Assessment and consultation are completed at our Sarnia office. The patient is evaluated and treatment options are discussed.

4. Communication back to you. Following the consultation or treatment, we provide a summary to the referring provider so you can maintain continuity of care for your patient.
